[00:01:07] Okay so we got stories to tell before we can actually talk about dental stuff.

[00:01:11] So turns out I have a long term connection to Dave.

[00:01:18] Yeah actually there's two of them.

[00:01:19] I mean I think your dad's known me since I was a kid as a patient.

[00:01:22] Probably baby teeth I'm thinking.

[00:01:24] Yeah, yeah.

[00:01:25] Yeah fully a way back to those 1970s rolled up jeans and popped up collars.

[00:01:30] Oh yeah.

[00:01:31] And middle school or good old central intermediate midland Michigan.

[00:01:35] You went to central?

[00:01:36] I did.

[00:01:37] Interesting.

[00:01:38] Then they made it up at Dow High though right?

[00:01:39] So central doesn't exist anymore.

[00:01:41] You know that story.

[00:01:42] No.

[00:01:43] Well there's an elementary school.

[00:01:44] It's Central Park Elementary.

[00:01:45] It's like the highest tech elementary school you've ever seen.

[00:01:47] It's crazy.

[00:01:48] My kid my kid so central they kept the auditorium.

[00:01:51] I was in the auditorium on Wednesday my first my son's concert.

[00:01:54] So yeah amazing.

[00:01:55] Yeah, did not know that this is podcast gold for people that don't know middle and Michigan yet here we are somewhere in Chicago.

[00:02:01] So yeah he went to the same high school that I went to and it's the same high school that my my son goes to.

[00:02:07] Wow.

[00:02:08] The A-Marches in the marching band is pretty amazing so.

[00:02:10] And my dad was Dave's dentist which is very hilarious.

[00:02:14] I'm sure I'm violating some kind of hippo by saying that but you know I want to now.

[00:02:24] So the other connection and this is like I've sort of known of Jake of Dave's like life because first off one guy by the name of Darin O'Brien

[00:02:34] he's been on the show it's been a while.

[00:02:35] I was in we were classmates in dental school.

[00:02:38] He's a lot different now than he was then.

[00:02:40] I mean at least his outward outwardly he's a lot different.

[00:02:43] I think probably to you you've probably seen the same Darin that I've seen.

[00:02:46] Yeah I would love to have seen Darin 20 years ago.

[00:02:49] But yeah I think I caught a glimpse of that literally only five years since I had known him for just 15-20 years now.

[00:02:55] Darin you know about Darin's nose right?

[00:02:57] Yeah.

[00:02:58] I pretty much let the whole world know about that but yeah I think it's story he doesn't love stuff.

[00:03:02] No he doesn't love it that's why I say whenever I possibly can't that's what I do.

[00:03:05] I will probably hear from him after this actually.

[00:03:07] I'm sure you will.

[00:03:08] Exactly but yeah.

[00:03:09] So yeah he was the other thing did you ever see Darin do any of the theatrical combat stuff?

[00:03:15] No if you've got video I don't think we do but I saw him.

[00:03:18] I saw it in person.

[00:03:20] I saw it in person.

[00:03:21] The dude had skills I'm telling you.

[00:03:22] It's too really.

[00:03:23] I do.

[00:03:25] I saw him as little John being tossed into the oh yeah.

[00:03:27] And to some of those Renaissance festivals.

[00:03:28] Yeah he was unreal.

[00:03:29] Minnesota Renaissance Festival was good.

[00:03:34] There are so many of us that would pay hundreds.

[00:03:36] He probably even loved to pretend that never happened.

[00:03:39] But it did I was there.

[00:03:40] I was there.

[00:03:41] If I disappear with weird circumstances I look at O'Brien first I'm just saying.

[00:03:46] During that guy that could either do that for you or do that too.

[00:03:49] Yeah he's got the skills that works in many ways.

[00:03:52] Of course I've known Rich Rosenblatt since the early dental town.

[00:03:55] I've known Rich Rosenblatt since before he was doing Sarax I'm pretty sure.

[00:03:59] So that just gives you one.

[00:04:01] I mean I went on dental town in 2002 I think I knew him from there.

[00:04:04] Wow.

[00:04:05] Yeah we were just talking to the dental town people over at the booth and how long

[00:04:08] and how fat you know how far back you can go back to people that are no longer

[00:04:11] even there.

[00:04:12] Yeah.

[00:04:13] You know I mean it's nice that Tom still there but she's how it's retired.

[00:04:15] Yeah yeah.

[00:04:16] Yeah.

[00:04:17] It's amazing how old we think about how old we were just talking with our men right?

[00:04:22] How nice and how old and I sometimes miss those days 20 years ago.

[00:04:27] It was a simpler time.

[00:04:28] I didn't have any gray hair.

[00:04:29] It was amazing.

[00:04:30] Yeah it was amazing.

[00:04:31] It was a small community though.

[00:04:32] I found that it's a really small close knit community which is really nice and you guys all

[00:04:37] have this camaraderie and so much about each other and there's a lot of respect so it's one

[00:04:42] thing I love especially about KKM down the street.

[00:04:44] Right.

[00:04:45] I'm going to argue that the podcast was built out of those people.

[00:04:48] If you go back to the early episodes there's all just all those people that I knew from

[00:04:52] the dental town and us.

[00:04:53] Yeah.

[00:04:54] So yeah we had to create our own space because that's how it works.

[00:04:56] And I say Ellen went to high school with my sister.

[00:04:58] Yeah.

[00:04:59] So yeah.

[00:05:02] It's like, so I mean I knew that you were a dentist at some point and then I realized then

[00:05:06] Rosenblatt must have dropped your name and like yeah it all came together.

[00:05:11] It's all comes, yeah sadly it's such a smog if it all comes around together whether we

[00:05:15] all want it to.

[00:05:16] Exactly.

[00:05:17] Exactly.

[00:05:18] That's right.

[00:05:19] So now we have to talk about dentistry.

[00:05:21] That's one of the things that we do here.

[00:05:23] Sadly.

[00:05:24] Yeah.

[00:05:25] So you guys have this is much more fun now.

[00:05:28] I mean, this is nobody cares.

[00:05:45] I know that because I was like five months sober.

[00:05:48] I was actually another recovering person pointed me at dental town and that was, you know,

[00:05:52] I got 22 years last month.

[00:05:54] So what's, just, just, 20.

[00:05:55] 20 pluses.

[00:05:56] Oh yeah.

[00:05:57] That's great.

[00:05:58] But I use it to know when I joined dental town.

[00:06:00] Let's be perfectly honest.

[00:06:01] That's right.

[00:06:02] So that's when I joined and that's when I got to know all these people.

[00:06:04] That was a long time ago.

[00:06:05] I can't be that old.

[00:06:07] I can't believe you stayed sober.

[00:06:08] Yeah.

[00:06:09] No, no, no, no, those people.

[00:06:10] No, exactly.

[00:06:11] That's the amazing, it's really easy to stay sober for 22 years.

[00:06:14] Yeah.

[00:06:15] It's a month away from those people.

[00:06:16] Exactly.

[00:06:17] Well, there was, I mean, like I said, I went to dental school with, with Derek.

[00:06:21] He's, a lot of people that have, you know, found me through the podcast have never known

[00:06:25] me in a time when I, when I would ingest those substances.

[00:06:28] I'm afraid O'Brien may have stories.

[00:06:30] I'm just going to say that right now.

[00:06:31] He's dental school for me.

[00:06:32] All right.

[00:06:33] Now back to teeth.

[00:06:37] So we're sitting here with two people who are very close with IVA Clark.

[00:06:40] They are in there's, did you guys roll out a new product at the midwinter?

[00:06:45] Yeah.

[00:06:46] So we're very excited Dr. Giuliani is here to introduce the next level of speed and

[00:06:50] strength in chair sides or cognizance.

[00:06:52] Okay.

[00:06:53] So I PSC Max or CAD Prime block.

[00:06:56] So it's a speed centred zirconia for chair side in 15 minutes with the aesthetics you

[00:07:01] would expect from IVA Clark and I think everybody's been waiting for us to have this moment

[00:07:05] and it's here.

[00:07:06] So it's very exciting.

[00:07:07] Okay.

[00:07:08] So people knew this was coming as what you're saying.

[00:07:09] They knew something like this was coming, is that right?

[00:07:13] It's tough to say I think there's been a lot of pressure.

[00:07:14] There's been a lot of questions as to whether we would, when we would but IVA Clark is

[00:07:19] known for being a company that works really hard to get it right before it comes to market

[00:07:23] and it might take us a little longer but we like to do things right.

[00:07:27] So it's exciting to see the ground swell of support behind us.

[00:07:30] It's fun.

[00:07:31] I don't think we needed it.

[00:07:32] I mean clinically as you know, we've been begging for an aesthetic and a strength in

[00:07:38] zirconia we just haven't gotten and this thing fits perfectly in the IVA Clark line.

[00:07:43] I mean the ability to have a zirconia that literally stays true to shade and matches

[00:07:51] all of the other aesthetic materials IVA Clark has.

[00:07:54] I mean I've done quad after quad of EMAX, zirconia and Empress multi and you can't see

[00:08:00] the difference.

[00:08:01] That's interesting.

[00:08:02] You could have a four year old point out that one doesn't fit right before zirconia

[00:08:05] so people were waiting for it not only because they kind of expected it to be IVA Clark but

[00:08:11] they needed it to be IVA Clark and they knew when it did finally get released it was going

[00:08:15] to be the product it is.

[00:08:16] And it's amazing.

[00:08:17] 15 minutes, we're talking 15 minutes in a really transitional block with translucency

[00:08:22] and strength that we control so you can position the rest of the block.

[00:08:27] Within the block.

[00:08:28] Yeah, that's true.

[00:08:29] Amazing strength for a second molar region.

[00:08:30] It goes anywhere from 800 so it's got three Y at the 800 at the cervical area all the

[00:08:35] way up to 800 and the five Y at the incisal and you get tons of translucency with it.

[00:08:40] So it really is, we were talking some of the developers of it today and it really was

[00:08:45] thought out like most IVA Clark products really, really well.

[00:08:48] Sure, yeah.

[00:08:49] Yeah, we actually have one of our furnace engineers who's here and he's doing lectures

[00:08:53] and I always say our material scientists work right there in the lab with the engineers

[00:08:59] and even the sintering furnace technology is so dialed in and we have this perfect portfolio

[00:09:05] of restorative work full solutions where you kind of have even a tetra crime composite

[00:09:11] next to Empress, next to Emacs and you can get that shade accuracy.

[00:09:15] That is really hard by the way.

[00:09:17] That's insane.

[00:09:18] That's not an easy thing to do.

[00:09:19] And your tetra direct composite.

[00:09:21] Yeah.

[00:09:22] You're right place composites.

[00:09:23] It's amazing.

[00:09:24] Yeah, his eyes lit up when he came to the booth and we were showing him people are so

[00:09:27] excited and the shade accuracy is just spot on and that tells a story because they're

[00:09:32] coignious tough.

[00:09:33] It's really tough to get what you're trying to get with the shades and I think it really

[00:09:37] delivers so yeah, it's fast which is great and then the bonus is 1100 megapascals of

[00:09:42] strength and then if you can get it looking really nice too, it's just icing on the cake

[00:09:46] and so.

[00:09:47] And I think the translucency truly with no characterization needed it blends in so nice

[00:09:52] to natural teeth.

[00:09:53] So it gives you such an opportunity to make those clinical decisions that you know you

[00:09:57] have to do every day for what's going to be not only aesthetic but what's going to

[00:10:02] be functionally.

[00:10:03] You know, the things we care about right is it going to be long lasting?

[00:10:06] Is it going to be functional?

[00:10:07] Is it going to withstand everything we need our patients miles to withstand?

[00:10:11] Do you think that Ken Dennis have I mean I'm not as keyed in as you guys are but like

[00:10:17] the ease of use with zirconia has to be like and of course the downside was zirconia

[00:10:22] always sort of looked like like zirconia for a long time.

[00:10:26] zirconia look like like you said, you can tell.

[00:10:29] So then the question is do you feel like adoption of zirconia in the cat game space?

[00:10:34] The chair side cat game space is now going to just take off because you got some looks

[00:10:38] like what they've been using the e-max on it's them right and I think what's happened

[00:10:42] is I think people it's come kind of full circle right from when you and I went to dental

[00:10:46] school.

[00:10:47] I think people love the idea of the advancements and ceramics where you could get aesthetics

[00:10:52] where pfms couldn't deliver but they also wanted back to the simplicity of the pfm

[00:10:56] whether it's a proper or delivery correct.

[00:11:00] So I think when zirconia came out they were willing I think we as a profession was willing

[00:11:05] to compromise aesthetics and diagnostics and so you give up so much diagnostic stuff

[00:11:11] with zirconia because you can't tell it's gold or vizirconia so yeah.

[00:11:15] I think now that they were willing to do that and we've kind of relaxed with the at least

[00:11:22] the comfort that it's here for a little while somebody finally had to work on the aesthetic

[00:11:26] side of it.

[00:11:28] And now the expectations are if it can be strong it should and we starting to demand as

[00:11:34] a profession the aesthetics from it so yeah I really do think that it's really come

[00:11:39] full circle where at least most of us on the cat campsite have been around for everything

[00:11:44] good and bad about it.

[00:11:46] The only thing I think we give up now at all is diagnostics and it's amazing.

[00:11:51] It's an amazing product that it's even convinced me so I have an interesting question.

[00:11:54] So like we know that pucks for big mills have kind of had this for a little while but you

[00:12:01] had to kind of miniaturize it a little bit.

[00:12:03] Can't tell it like has that been the challenge to miniaturize it?

[00:12:07] No, I do want to go back to your last question though because you asked about adoption

[00:12:12] of zirconia and I think we thought it would be a lot faster than it has been.

[00:12:16] It was really fast for a few years there and we're starting to see a stabilization

[00:12:20] so e-max is still I mean it's our core product it's our brand so it's not going anywhere.

[00:12:27] Sure, yeah that's worth we're saying for sure yeah yeah and it's been really interesting

[00:12:32] to see that the speed was really a primary driver of zirconia now that we have the CS6 furnace

[00:12:38] you can crystallize it in 11 minutes it's really neutralized that conversation.

[00:12:42] Yeah yeah yeah it sort of makes it makes it the your choices have changed a bit.

[00:12:47] Yeah 100 percent definitely.

[00:12:49] If I didn't want to miss that one.

[00:12:50] No, that's worth mentioning.

[00:12:51] I am curious though like the technology I feel like we've seen the translusions with

[00:12:55] strength in a full-size puck for am I right for a little while longer.

[00:12:59] So then the call was to try and make it so you could do a chair side in a block.

[00:13:03] Right so there's a difference with the puck technology you traditionally have had

[00:13:09] to because you have a combination of powders so you have the 3Y, 4Y5Y some combination

[00:13:14] of those and they have their own centering rates.

[00:13:17] In order to put them in one centering program we had to have a longer cycle with the puck

[00:13:22] materials so being able to put that in a chair side application requires really much more

[00:13:29] scientific way of saying this but it's really a doping of the components to have them

[00:13:34] have the same centering rates because they don't work.

[00:13:37] Yeah yeah.

[00:13:38] Because that's the biggest issue you have especially when you get into longer spans

[00:13:40] yeah bigger frustrations.

[00:13:42] So it's remarkable to see what the team has done and they just they actually just presented

[00:13:47] this at the IDR in Rhodes Greece last year.

[00:13:50] They figured out the proper centering rates to get that into a speed application.

[00:13:55] That's really cool and I think people forget that Zirconia looks amazing but it needs 9

[00:14:00] to 16 hours of centering so for all of us the minute they've committed to a chair side

[00:14:06] product that's going to look great.

[00:14:08] They also have to commit to our time constraints on that and the efficiency that we require

[00:14:13] in a 15 minute center so I think it's not the nice thing about the puck is it was easier

[00:14:19] because those are mostly lab generated products so you could still get such an amazing thing

[00:14:24] because they can still center for the transfer.

[00:14:26] Yeah they can still see it for that time so literally yeah.

[00:14:28] Yeah truly this block honestly I think it to me if you take just the normal average every

[00:14:34] day I even some of us have trained you cannot tell the difference between this block on the

[00:14:40] on a table in some other longer term center translucent that's really cool Zirconia

[00:14:45] blocks and it's amazing what this oven can do in 15 minutes not only for the 15 minute

[00:14:50] centering cycle but the 11 minutes of E max I mean you and I were there with compact

[00:14:54] milling when E max came out was 28 minutes and you're firing it forever and just amazing

[00:14:59] how much I mean the draw in the just the compelling

[00:15:04] the level of compelling for chair side all of a sudden gets even you know it's gotten

[00:15:08] more and more compelling for people who are her own for about the time you know with

[00:15:13] with chairs that's really cool.

[00:15:14] It's exciting yeah.

[00:15:15] Well we just got our patent approved on the CSX technology with the vacuum assist so

[00:15:21] it's the first centering furnace in the world to have vacuum assist with fresh oxygen flush

[00:15:26] oh well.

[00:15:27] And it actually increases the transducency of restorations and it makes them very vibrant

[00:15:33] as well so we can do more conservative dentistry for instance using a Zirquette out he restoration

[00:15:40] because we can make it more lifelike more translucent and you still get that point

[00:15:44] six millimeters of reduction and you can go more conservative and you can have a beautiful

[00:15:49] restoration.

[00:15:50] Actually I was telling you Clint Stevens just did a rota paper a he and Molly related

[00:15:54] some research comparing aesthetics of an S2 fire to the lab centering furnace firing

[00:16:02] restorations versus the CS6 chair side and they're very comparable in transducency

[00:16:07] and aesthetics.

[00:16:08] And we come so far chair side it's really exciting.

[00:16:11] I think that's what's so exciting about I mean obviously everything we get to do now

[00:16:14] and see I can't imagine those 75 80 years where you just came in and plugged teeth with

[00:16:20] an album yeah and we get to be a part of the things that literally are shaping the aesthetics

[00:16:26] I mean everybody in this meeting and everybody in dentistry at this time is seeing so many

[00:16:31] amazing things that you and I you know didn't know for the first 10 years of our career so

[00:16:36] it was really a nice it's really exciting to be a part of the lot.

[00:16:39] So you've had a chance to like use this for a little while.

[00:16:41] I have.

[00:16:42] I want you to just tell them so this is for the people that are probably don't understand

[00:16:47] necessarily a lot of the high level how has this changed the way that you are going

[00:16:53] to prep teeth?

[00:16:54] How has this changed the way that you're going to use them too?

[00:16:57] Like if you are you know you're working on a regular quadrant of post year restorations

[00:17:02] what's different how are you doing this differently and how are you going to plan differently?

[00:17:05] It actually completely opened up a lot of our older chair side users would automatically

[00:17:11] think EMAX in a high strength.

[00:17:13] Yeah, high area need whether you need a good function prepping was always the challenge

[00:17:19] there because you just didn't have the room.

[00:17:21] So what this is done is it's completely opened up any high you know high strength need

[00:17:26] to another material you can be conservative on your props for so many reasons retention

[00:17:31] aesthetics you know, pulple issues and it is really definitely my quadrant based whole

[00:17:38] workflows changed actually a little bit for the better because now you have this Erkonea

[00:17:42] product that comes out of the milling chamber goes right into the oven.

[00:17:45] You get your EMAX out next so it's really made me think about not having to compromise

[00:17:51] the aesthetic sometimes for for strengths where I used to have to do that was the decision

[00:17:56] I would make every day.

[00:17:57] I need strength here so I'm going to have to give up aesthetics and there are days now

[00:18:01] I'll go right to the EMAX or CAD prime where before I would think alright I may have

[00:18:07] to just use EMAX on this situation where I'll get my aesthetics but I don't know if I'm

[00:18:10] going to have the prep geometry I need or the reduction I'm going to need to get for full

[00:18:14] strength on the EMAX in this down to I mean you can go down to .8 on this as ideal but

[00:18:20] in some of the three Y areas of the block you can go down to 6.6 millimeters.

[00:18:24] So my mind goes to a second molar situation 100 just you just die in for room and you

[00:18:30] get out exactly that's cool.

[00:18:32] And I mean that was always not a weakness per se but but chair side CAD cam that was

[00:18:38] a struggle when you were working on that particular patient you know.

[00:18:42] I mean I heard people sing the blues about that it's nice to know that you've got that's

[00:18:47] I mean that's not very much reduction you can get away with quite a bit it's on to

[00:18:50] it.

[00:18:51] Proposals look pretty nice too.

[00:18:52] And they do.

[00:18:53] That's cool.

[00:18:54] And today really no matter what material we're getting really true and product from what

[00:18:59] we see on the screen.

[00:19:00] And I think you were there earlier on where sometimes you'd look at the screen and you'd

[00:19:03] look at the unproduct thinking yeah yeah I'm not sure where this went wrong but yeah

[00:19:07] it's a bait and switch thing but yeah this is truly I think the soft we're finally at

[00:19:11] that point I think where we always knew it would come where the materials the hardware

[00:19:16] the software all finally met.

[00:19:18] Yeah.

[00:19:19] And we're there now with amazing choices that gives us the extra you know the aesthetics

[00:19:23] and the strength that we need and our patients need we demand yeah and it works out really

[00:19:27] well.

[00:19:28] It's really exciting.

[00:19:29] It's really exciting so.

[00:19:30] Should you tell them about the next thing coming out exclusive top secret I could tell

[00:19:33] you but you had I'd have to kill you.

[00:19:35] So Dr. Giuliani has also been testing another product that fits into this workflow that's

[00:19:40] coming soon which is our new polishing system.

[00:19:43] Yeah so I didn't know if I could mention that.

[00:19:46] I can tell you can I really say it right now.

[00:19:48] I know it got to be a lot of people coming for it.

[00:19:51] I'll be like arrested right when I go to the room after the end you broke it yeah you're

[00:19:55] done.

[00:19:56] It's cats out of the bag yeah so we're going to be releasing a new opticals extra oral

[00:20:00] posture so it's going to be coming soon and they just work so beautifully with this

[00:20:04] in that you can I think they increase your shade accuracy and Dr. Giuliani can speak

[00:20:09] to that but we're excited about that too.

[00:20:10] And it's nice because I think everybody gets a block like this you know it's when EMAX

[00:20:13] first came out what do you it's blue what do you do with it.

[00:20:17] This one's white.

[00:20:18] And I think that's the one thing about it is the finishing of this product was well

[00:20:21] thought out too.

[00:20:22] You do a pre polish with the same things you're doing final polish with and unlike most

[00:20:27] erkonia you really are working hard to make it look bad.

[00:20:32] You have to work really hard to have this look like a pearl you know almost erkonia

[00:20:35] is very pearl I said looking hard to make it look bad.
