[00:03:11] [SPEAKER_03]: Just a little bit about myself.

[00:03:13] [SPEAKER_03]: I graduated in 2005.

[00:03:15] [SPEAKER_03]: I'm a general dentist in a very rural area of eastern North Carolina.

[00:03:19] [SPEAKER_02]: Just bread and butter, general dentist.

[00:03:22] [SPEAKER_02]: Cool.

[00:03:23] [SPEAKER_02]: And also we have Kevin Fryer with us today.

[00:03:25] [SPEAKER_02]: Hey, Kevin, how's it going?

[00:03:26] [SPEAKER_04]: Going good. Glad to be here, Zach and Mac.

[00:03:30] [SPEAKER_04]: I graduated in 1989 from the Ohio State University.

[00:03:35] [SPEAKER_02]: Got to get that V in there.

[00:03:36] [SPEAKER_04]: And I practice in a suburb of Cleveland, Ohio.

[00:03:39] [SPEAKER_04]: I've been there for 30 years.

[00:03:41] [SPEAKER_02]: Do you have your own practice, Kevin?

[00:03:44] [SPEAKER_04]: Yeah. Just me, solo.

[00:03:47] [SPEAKER_04]: Basically bread and butter,

[00:03:49] [SPEAKER_04]: but we do emphasize some implants, ortho, and sleep.

[00:03:53] [SPEAKER_02]: Cool.

[00:03:54] [SPEAKER_02]: I'm Zach Miners.

[00:03:55] [SPEAKER_02]: I graduated from University of Missouri-Kansas City in 2001.

[00:04:00] [SPEAKER_02]: I still work with my dad,

[00:04:02] [SPEAKER_02]: who also graduated from UMKC back in the 70s.

[00:04:08] [SPEAKER_02]: That's the nutshell of who we are.

[00:04:12] [SPEAKER_02]: We're just going to start picking topics week to week,

[00:04:15] [SPEAKER_02]: sometimes on a product, sometimes on something new,

[00:04:18] [SPEAKER_02]: just sometimes on a technique.

[00:04:21] [SPEAKER_02]: We thought it would be super sexy to start off with just

[00:04:24] [SPEAKER_02]: the composite brands we use in our office on a day-to-day

[00:04:28] [SPEAKER_02]: and why we picked them and what they are

[00:04:31] [SPEAKER_02]: and what we like or dislike about them.

[00:04:35] [SPEAKER_02]: I'm going to start off with Mac,

[00:04:37] [SPEAKER_02]: simply because he's sitting on my right here.

[00:04:40] [SPEAKER_02]: Mac, tell me about your composite situation

[00:04:43] [SPEAKER_02]: that you've got going on in your office.

[00:04:44] [SPEAKER_02]: What do you use?

[00:04:46] [SPEAKER_03]: It's maybe a little more complex than it should be,

[00:04:48] [SPEAKER_03]: but I'm a big BioClear guy, so a lot of 3M products.

[00:04:51] [SPEAKER_03]: We're using the Fil-Tec 1 for our Class 2s,

[00:04:55] [SPEAKER_03]: Fil-Tec Supreme for our anteriors.

[00:05:00] [SPEAKER_03]: About a year ago, I probably had 15 different composite brands,

[00:05:02] [SPEAKER_03]: and I've tried to whittle that down,

[00:05:04] [SPEAKER_03]: but I still have a lot of CosmoNet products.

[00:05:06] [SPEAKER_03]: I like their Micro-Fill,

[00:05:07] [SPEAKER_03]: and I'll dust that off for certain situations.

[00:05:10] [SPEAKER_03]: Maybe pair that with a Micro-Hybrid for a Class 4.

[00:05:14] [SPEAKER_03]: Also use GC Genial Sculpt.

[00:05:17] [SPEAKER_03]: That's your flowable?

[00:05:18] [SPEAKER_03]: No, that's a paste.

[00:05:19] [SPEAKER_03]: Just a paste.

[00:05:20] [SPEAKER_03]: That was my go-to Class 2 for years

[00:05:22] [SPEAKER_03]: once I adopted the microbrush technique

[00:05:24] [SPEAKER_03]: Jason Smithson teaches.

[00:05:27] [SPEAKER_03]: I used that for a long time,

[00:05:29] [SPEAKER_03]: but once I went BioClear, it's pretty much all 3M Fil-Tec.

[00:05:32] [SPEAKER_02]: Do you use any flowable composites in your—

[00:05:34] [SPEAKER_03]: With the BioClear, yes.

[00:05:36] [SPEAKER_03]: You use a heated flowable,

[00:05:37] [SPEAKER_03]: which is also a Fil-Tec product.

[00:05:40] [SPEAKER_04]: What's the name of that, the flowable?

[00:05:42] [SPEAKER_03]: I think it's also Fil-Tec Supreme, maybe.

[00:05:44] [SPEAKER_02]: Flowable.

[00:05:45] [SPEAKER_02]: You use the bulk fill on most of your molars

[00:05:48] [SPEAKER_02]: and premolars, your Class 1s and 2s,

[00:05:50] [SPEAKER_02]: and then you switch when you're doing the front to—

[00:05:53] [SPEAKER_03]: If I'm doing BioClear in the anterior,

[00:05:55] [SPEAKER_03]: it's also a bulk fill.

[00:05:57] [SPEAKER_03]: Depth of cure is not as much of an issue

[00:05:58] [SPEAKER_03]: because you can cure from the buccal and lingual

[00:06:00] [SPEAKER_03]: on that situation pretty well.

[00:06:02] [SPEAKER_03]: You need the aesthetics of a non-bulk fill there.

[00:06:05] [SPEAKER_03]: It's going to be way too translucent

[00:06:06] [SPEAKER_03]: doing an actual bulk fill there, but yeah.

[00:06:11] [SPEAKER_02]: For the BioClears, which I'm not currently doing,

[00:06:14] [SPEAKER_02]: how many shades of this stuff

[00:06:16] [SPEAKER_02]: do you have to keep around to make everything happen?

[00:06:19] [SPEAKER_03]: That is a very good point,

[00:06:20] [SPEAKER_03]: and that's one of the things I really like about the system.

[00:06:23] [SPEAKER_03]: When I first got back from Tacoma,

[00:06:25] [SPEAKER_03]: I did a few cases, posted some photos,

[00:06:27] [SPEAKER_03]: maybe in one of the dental hacks groups,

[00:06:28] [SPEAKER_03]: got some feedback actually from David Clark himself.

[00:06:32] [SPEAKER_03]: I think I was doing like an A3 or something,

[00:06:35] [SPEAKER_03]: which used to be my standard Class 2 posterior shade.

[00:06:39] [SPEAKER_03]: They were looking pretty great in that bulk fill.

[00:06:43] [SPEAKER_03]: He gave me a great tip.

[00:06:44] [SPEAKER_03]: He said anything in the posterior, he uses B1.

[00:06:49] [SPEAKER_03]: In the BioClear method, B1 looks great.

[00:06:53] [SPEAKER_03]: That literally stopped one shade of posterior composite,

[00:06:56] [SPEAKER_03]: and that's B1.

[00:06:57] [SPEAKER_02]: Every posterior filling, whether they're A5 or A1,

[00:07:02] [SPEAKER_02]: gets B1 in your hands.

[00:07:04] [SPEAKER_02]: B1, it looks great.

[00:07:05] [SPEAKER_04]: All of it's heated.

[00:07:07] [SPEAKER_02]: All of it's heated.

[00:07:08] [SPEAKER_02]: That's the key.

[00:07:09] [SPEAKER_02]: How about for the anterior?

[00:07:10] [SPEAKER_02]: Do you have the full shade guide worth of shades

[00:07:14] [SPEAKER_02]: for your Supreme Ultra?

[00:07:15] [SPEAKER_03]: I do not.

[00:07:17] [SPEAKER_03]: I don't know how many shades we have,

[00:07:19] [SPEAKER_03]: but I'm in a unique location.

[00:07:23] [SPEAKER_03]: It's very rural.

[00:07:24] [SPEAKER_03]: People are literally looking for something to fill.

[00:07:26] [SPEAKER_03]: Black spaces have got a chipped tooth.

[00:07:29] [SPEAKER_03]: I've had these conversations with my associate.

[00:07:32] [SPEAKER_03]: Do we do a lot of layering and stuff?

[00:07:34] [SPEAKER_03]: Are we really doing that for the patient,

[00:07:35] [SPEAKER_03]: or are we doing it for ourselves?

[00:07:36] [SPEAKER_03]: For our fellow dentist to post a trophy case on Facebook.

[00:07:42] [SPEAKER_03]: The ability of a composite,

[00:07:44] [SPEAKER_03]: and if you've got a nice bevel in there,

[00:07:45] [SPEAKER_03]: which you're going to need with BioClear,

[00:07:47] [SPEAKER_03]: it's going to blend in.

[00:07:49] [SPEAKER_03]: It's going to look great.

[00:07:50] [SPEAKER_03]: It's going to meet the needs of your patient.

[00:07:52] [SPEAKER_03]: They want the bite to feel well.

[00:07:53] [SPEAKER_03]: They want it to feel smooth,

[00:07:54] [SPEAKER_03]: and they don't want to stick out like a sore thumb.

[00:07:57] [SPEAKER_02]: I know that particular composite comes in body

[00:08:01] [SPEAKER_02]: and enamel shades.

[00:08:02] [SPEAKER_02]: Do you just stock the body shades?

[00:08:04] [SPEAKER_02]: I think it's just the body shades we have.

[00:08:05] [SPEAKER_02]: Just the body shades,

[00:08:06] [SPEAKER_02]: and a few different of the A's,

[00:08:08] [SPEAKER_02]: and call it a day.

[00:08:08] [SPEAKER_02]: Okay, cool.

[00:08:10] [SPEAKER_02]: Kevin, what's your composite story in your place?

[00:08:13] [SPEAKER_04]: Currently using a product called Calor.

[00:08:16] [SPEAKER_04]: It's a GC product,

[00:08:17] [SPEAKER_04]: and that's all I use,

[00:08:19] [SPEAKER_04]: anterior and posterior.

[00:08:20] [SPEAKER_04]: It's a holdover from a previous employee,

[00:08:23] [SPEAKER_04]: which that's another conversation.

[00:08:24] [SPEAKER_04]: Sure.

[00:08:28] [SPEAKER_04]: That's basically what I use.

[00:08:30] [SPEAKER_04]: I carry, I think,

[00:08:32] [SPEAKER_04]: probably four or five shades.

[00:08:35] [SPEAKER_04]: Some A1, A2, B1,

[00:08:41] [SPEAKER_04]: and some opaque shades for the anterior.

[00:08:48] [SPEAKER_04]: Like we talked previously before we went on air,

[00:08:52] [SPEAKER_04]: I like it, I don't love it.

[00:08:54] [SPEAKER_02]: How about flowables?

[00:08:56] [SPEAKER_02]: Do you use this Calor have its own flowable?

[00:08:58] [SPEAKER_02]: I do not.

[00:08:59] [SPEAKER_02]: I use an SDR product for lining the boxes.

[00:09:06] [SPEAKER_02]: Is that a bulk fill flowable?

[00:09:08] [SPEAKER_02]: It's like the first layer?

[00:09:09] [SPEAKER_02]: Do you just put that down as your first couple millimeters?

[00:09:13] [SPEAKER_04]: Yeah, I put that into the box.

[00:09:16] [SPEAKER_02]: Yeah, tell me more.

[00:09:19] [SPEAKER_04]: Slowly at first, just the tip.

[00:09:21] [SPEAKER_04]: I try to line the box up to about the pulpal floor.

[00:09:28] [SPEAKER_04]: It has a self-leveling property,

[00:09:31] [SPEAKER_04]: and it just fills in that space,

[00:09:34] [SPEAKER_04]: and then we cure that,

[00:09:36] [SPEAKER_04]: and from there we do a layering technique.

[00:09:41] [SPEAKER_02]: The anterior,

[00:09:43] [SPEAKER_02]: you use the same stuff, same everything.

[00:09:46] [SPEAKER_02]: Do you like it?

[00:09:46] [SPEAKER_02]: Do you like this material or are you just meh on it?

[00:09:49] [SPEAKER_04]: I'm sort of indifferent on it.

[00:09:52] [SPEAKER_04]: You could convince me to change to something else.

[00:09:57] [SPEAKER_04]: Actually, a couple months ago,

[00:09:59] [SPEAKER_04]: I was really looking hard for a different composite material.

[00:10:05] [SPEAKER_04]: Part of it is though, strictly from a business standpoint,

[00:10:07] [SPEAKER_04]: it's just easy to order.

[00:10:10] [SPEAKER_04]: And switching to something,

[00:10:11] [SPEAKER_04]: and then having all this extra stuff,

[00:10:13] [SPEAKER_04]: I hate that.

[00:10:15] [SPEAKER_04]: The inventory is an issue.

[00:10:17] [SPEAKER_04]: So we recently switched some of our cements,

[00:10:23] [SPEAKER_04]: and we had an inventory of stuff.

[00:10:28] [SPEAKER_04]: I'm like, I'm not ordering this new stuff

[00:10:30] [SPEAKER_04]: until we've got everything out of the way and moving forward.

[00:10:33] [SPEAKER_04]: So that's been an issue.

[00:10:34] [SPEAKER_04]: That's just a hang-up.

[00:10:36] [SPEAKER_04]: But yeah, I could be convinced to change.

[00:10:41] [SPEAKER_02]: Either one of you guys stock any alternative filling materials

[00:10:45] [SPEAKER_02]: like glass ionomers or dual cure resins for buildups,

[00:10:53] [SPEAKER_02]: any of those types of things in your arsenal?

[00:10:55] [SPEAKER_03]: I do a lot of glass ionomers.

[00:10:56] [SPEAKER_03]: I'm a big Fuji II LC guy.

[00:10:59] [SPEAKER_03]: For any Karius Class V, that's pretty much what I'm using.

[00:11:02] [SPEAKER_03]: You kind of turned me on a few months ago to the Equia Forte.

[00:11:05] [SPEAKER_03]: I love that product.

[00:11:07] [SPEAKER_03]: Which I will use to get a little bit more fluoride release.

[00:11:10] [SPEAKER_03]: It's just so slow setting.

[00:11:12] [SPEAKER_03]: In certain situations, I just want to go ahead and hit it with that light

[00:11:14] [SPEAKER_03]: and be able to go ahead and finish it back.

[00:11:17] [SPEAKER_02]: Try thermo curing it for a minute with your light.

[00:11:20] [SPEAKER_02]: The light doesn't do anything, but the heat does.

[00:11:22] [SPEAKER_03]: Are you doing that immediately or are you giving that two-minute window?

[00:11:25] [SPEAKER_02]: I give it a minute or so, and then I'll hit it with the thermo cure.

[00:11:29] [SPEAKER_02]: It still is a little time consuming.

[00:11:31] [SPEAKER_02]: That is a downside to that.

[00:11:32] [SPEAKER_03]: I will do that, and I have found if I go do a hygiene check

[00:11:35] [SPEAKER_03]: and come back a few minutes later,

[00:11:37] [SPEAKER_03]: the finishing is so much better than if I do it right after that thermo cure.

[00:11:41] [SPEAKER_03]: Huge better.

[00:11:41] [SPEAKER_03]: But I'm very impatient.

[00:11:42] [SPEAKER_04]: How long does it take?

[00:11:44] [SPEAKER_04]: Do they say in the instructions?

[00:11:45] [SPEAKER_02]: It's a five-minute set.

[00:11:48] [SPEAKER_02]: That's an eternity.

[00:11:50] [SPEAKER_02]: It is an eternity.

[00:11:51] [SPEAKER_02]: It's put it in, and then you have to make sure it's set up enough

[00:11:54] [SPEAKER_02]: that it's not just going to slump out.

[00:11:56] [SPEAKER_02]: Then you got to go do a hygiene check, and you got to come back.

[00:11:58] [SPEAKER_02]: For some people, that doesn't work into their workflow of their day.

[00:12:02] [SPEAKER_02]: Dual cure stuff for buildups, Mac?

[00:12:05] [SPEAKER_03]: I'm a big Apex guy.

[00:12:07] [SPEAKER_03]: Surpassed John Kanka's products.

[00:12:09] [SPEAKER_03]: I use his Anchor as a dual cure buildup.

[00:12:11] [SPEAKER_03]: That's your buildup material.

[00:12:12] [SPEAKER_03]: Also love Cosmic Core.

[00:12:15] [SPEAKER_03]: To me, they're sort of all the same.

[00:12:17] [SPEAKER_03]: You can tell a little bit of a difference cutting some of them,

[00:12:20] [SPEAKER_03]: but they all fit the bill.

[00:12:22] [SPEAKER_03]: I'll use Fuji too for a lot of buildups or Equia.

[00:12:25] [SPEAKER_03]: Actually, the old silver Miracle Mix.

[00:12:28] [SPEAKER_03]: That's kind of a product that gets pooped on a lot,

[00:12:31] [SPEAKER_03]: but God, it's worked great in my practice.

[00:12:33] [SPEAKER_03]: If I've got a deep margin,

[00:12:34] [SPEAKER_03]: and I want to see that difference to two structures,

[00:12:37] [SPEAKER_03]: it's a great product for that.

[00:12:38] [SPEAKER_02]: I have never used Miracle Mix, so I know what you're talking about.

[00:12:39] [SPEAKER_02]: For those of you listening,

[00:12:41] [SPEAKER_02]: Miracle Mix is a glass polymer with silver infused into it.

[00:12:46] [SPEAKER_04]: I used that when it was a new product.

[00:12:49] [SPEAKER_04]: Nice.

[00:12:49] [SPEAKER_04]: But I've gone away from that.

[00:12:53] [SPEAKER_03]: I've had patients that have a period-involved tooth.

[00:12:56] [SPEAKER_03]: They just don't want it out yet, and they break a cusp off.

[00:12:58] [SPEAKER_03]: And I'll just squirt Miracle Mix there,

[00:13:00] [SPEAKER_03]: and finish that back, and replace that cusp.

[00:13:02] [SPEAKER_03]: I've had them last like 10 years.

[00:13:04] [SPEAKER_03]: They're still there.

[00:13:06] [SPEAKER_04]: I had a patient whose crown came off,

[00:13:09] [SPEAKER_04]: and I used Miracle Mix probably in 1992.

[00:13:12] [SPEAKER_04]: Yeah.

[00:13:12] [SPEAKER_04]: And I looked at that prep, and the Miracle Mix,

[00:13:15] [SPEAKER_04]: and I was like,
