[00:04:15] Tell us how you got into this YouTube world of dentistry that you've been so successful with.

[00:04:21] Yeah, you know again I can't say how grateful I am to be part of what you guys are doing

[00:04:26] and it's a really neat thing you guys are doing.

[00:04:28] So kudos to the two of you for putting it together.

[00:04:32] I'll be honest, I actually shower because I thought this was going to be a video thing

[00:04:36] so I'd showered into my hair.

[00:04:37] But sorry about that man.

[00:04:45] But it turns out that we're doing it just audio so I was like okay.

[00:04:56] So yeah, so I think it started about 2011 and I was doing a US Army Advanced Education

[00:05:04] General Dentistry Residency and after my first year I learned so many amazing things that I was

[00:05:11] talking to.

[00:05:12] I talked to my boss and I was like well can I record this stuff?

[00:05:18] And they said sure.

[00:05:20] And I said well you know there's two reasons.

[00:05:22] One is I want to remember what the procedures are in about 10 years when I

[00:05:27] have more children and forget what I'm doing if the thing hasn't changed.

[00:05:31] And also I want to share the basic simple simple simple things like you know using Vaseline

[00:05:38] on a crown when you adjust the occlusion.

[00:05:41] You know, simple simple things that make such a big difference so that started then and then

[00:05:46] here we are now.

[00:05:48] That's about it.

[00:09:47] Okay so Ash you have produced a really nice three video series lately on calcify canals

[00:09:56] and navigating calcify canals.

[00:09:58] It's a good solid like what probably 150 minutes were the content?

[00:10:02] You didn't nobody watches it except for the intro.

[00:10:05] So anyway if we can just get you to condense that down to about 15 minutes

[00:10:13] in an audio format.

[00:10:15] That's awesome.

[00:10:18] I got another 50 minute or just I was thought we're going to screen capture too

[00:10:22] to share with you but apparently we can't so.

[00:10:29] What is tell me just what are some of the low hanging fruit tips for you get into a tooth

[00:10:37] and calcified and you're trying to make it happen we just give it give us

[00:10:41] something like the real low hanging fruit tips to get us started on the basic ones.

[00:10:45] Yeah of course so and a lot of these just before I get started a lot of these most of them

[00:10:50] and taught to me that's all not me sitting around like wow this magician it's not that it's

[00:10:56] just been taught by other people so the first one I can say before we even get started is make

[00:11:02] you sure that we have the right diagnosis that's pretty cheesy.

[00:11:06] Yeah but I do a lot of um we teach a lot of new dentist now and uh uh it's I don't know

[00:11:14] we're uh just need to get the right diagnosis.

[00:11:17] So the second one is you always take a period of a great graph that's standard but

[00:11:22] you know when you take the PA and you look at it and you're like whoa that's super calcified like

[00:11:26] oh my gosh I don't want to get in that but uh I don't know if you guys have been using

[00:11:30] if you ever use a bite wing I'm sure you have yeah always take a vertical bite wing for any

[00:11:35] endo absolutely you gotta have a great uh you know that's a pretty standard tip that's great

[00:11:41] to hear do you guys use combbeams for these things now too?

[00:11:44] I do yes. I don't have a combbeam Kevin Kevin does so uh it sounds awesome it sounds like

[00:11:50] it's very helpful and you've been uh you know what kind do you have and

[00:11:55] I have the the serona combbeam um and I tell you what it's really useful for

[00:12:02] pre-molars in particular if you're not sure if there's one or two canals I mean it's no

[00:12:06] it's a no brainer you know that it's there um today I actually had a single rooted second molar

[00:12:12] and it looked like it on the PA and uh I just wanted to make sure because you absolutely

[00:12:18] that could be a really weird tooth right getting into it and to be able to take the combbeam and

[00:12:23] just drill down from the occlusal and see that it really was just one canal um for most of my career

[00:12:30] that would have been a mystery to me I would have been looking around for other canals because

[00:12:34] they're supposed to be there and just to know that right off the bat I mean that that appointment took

[00:12:40] you know a half hour and um you know it was it was uh really infected and um was actually I heard

[00:12:48] you have a video of pus straining out of a tooth I could have made a video like that today

[00:12:53] so we were just gonna open it up and close it but just to be able to know that there was really

[00:12:58] actually one canal there from the combbeam was very uh useful and and a time saver

[00:13:04] and probably a tooth saver because I probably would have rooted around for something that wasn't

[00:13:07] there and you know perforated or something sure yeah absolutely that's uh you know so that's a you

[00:13:13] know those three those two things you combbeam and if you don't have a combbeam um just the uh the

[00:13:18] vertical bite wing uh and then the next one is probably one of the biggest things especially for

[00:13:23] molars um say you're doing calcified molar all one of the things I'd listen to um I don't

[00:13:30] know if you've watched John Rhodes his uh oh yeah his definitely pimped his channel huge yeah

[00:13:36] really good uh you guys should talk to him he's a super nice guy I did a talk with him he is

[00:13:42] super nice guy and then uh so we were talking about calcified cases and there was I think he was

[00:13:48] talking with me or if you had a video but he was saying like not all super calcified canals are

[00:13:54] calcified they're just full of junk that seem to be radio opaque and then when you get in you're like

[00:13:58] oh just kind of like slither's in so I think that that's one of the things that I've learned is

[00:14:03] that even if the chamber looks fairly calcified that there is going to be some sort of space most

[00:14:08] of the time you just got to get to it and then especially molars using the uh doichim music

[00:14:15] cant rule of six millimeters you guys familiar with that oh learning so six millimeters they've in

[00:14:21] 2002 2004 they published an article that talks about uh on most on all actually molars and

[00:14:31] mandible their molars the tie the pulp chamber the roof stops at six millimeters from the

[00:14:37] mesobuckle cuss say for example so if you drop on the dimension of a burr I don't have a burr on

[00:14:43] me it's okay dimension of a burr so that where the shank changes from the tip to where the shank

[00:14:51] changes is six mills so what I'll do is that what it does is it gives you that quantitative measurements

[00:14:57] you can drop right down to where that shank changes and then feel comfy like oh um i'm not

[00:15:04] perfect anything because the numbers really are six millimeters to the top of the pulp chamber

[00:15:09] you got two millimeters of pulp chamber width a height and then eleven millimeters into the

[00:15:14] frication okay so i don't know are you guys using long shank burrs i use regular shank most of

[00:15:20] the time i it's a it just depends on the tooth i have them available but i mean i start with i

[00:15:26] start with short yeah so what i'll do is i used to think that the long shank may you you know

[00:15:33] helped you get to the frication faster so i never used them ever until i know i realize

[00:15:40] until somebody told me like it pushes the head of your hand piece out of the way so you can see

[00:15:44] what the hell is going on yeah yeah it really i mean i wish i could show you video on this thing but

[00:15:52] the it's just it's so remarkable just being able to see what you're doing this is incredible

[00:15:58] so i get that out of the way so what happens is that if you open up to the six mill at the top

[00:16:03] of the pulp chamber that allows you to kind of like wash it out take a look i don't know if you guys

[00:16:09] are using loops or a scope absolutely for sure loops yeah kevans are like what six power loops

[00:16:16] or five five yeah i'm it's i'm in four oh i know you probably use a microscope because i've seen

[00:16:22] the i've seen you use your i've seen your your skills your scope on audio videos so yeah

[00:16:27] it's a cheater way to collect video yeah sorry recording right it's really easy yeah you just

[00:16:35] click record what power what power do you usually put your scope on to both do your procedures or

[00:16:40] do your videos or either one yeah so that's such a great question you know like four times is at the

[00:16:46] end of the like where the rubber hits the road it's like if it's a four times because there's like

[00:16:52] all the buy-nose also there's detailed information about what the mag is in every scope a little

[00:16:58] bit different but usually it's at four six times so my experience has been with like four or three

[00:17:04] and a half loops you're good to go do an endo if you're trying to search for something that's really

[00:17:10] deep down it's gonna be pretty tough without when i was overseas using 3.5s it worked amazing

[00:17:18] with a great light so yeah that's what i'm sure you guys are you're saying you're using fours

[00:17:24] and five which is say you can i remember my buddy i was in the Halifax he gave him this nickname

[00:17:31] himself he's a prostitut honest and he if you ever listened to this he made this nickname like

[00:17:36] you know you have like you're in an audience you put names on your phone for an app to ask questions

[00:17:42] and he's the only prostitut honest in the room and he puts on prosto daddy like buddy we

[00:17:50] we know who you are uh mechlos anyways uh so he said he i remember when he first got a

[00:17:57] residency he was with me and he got eight times loops or seven and i'm he's like he could see his

[00:18:04] heartbeat because yeah that's hilarious that's good there's some pretty detailed margins

[00:18:14] absolutely so go on oh go ahead so what you're all right so you've got your you've got your

[00:18:21] kind of your access done on that like once we start getting kind of into the canals or any

[00:18:26] what's your like best pro tips for starting to negotiate some that calcified areas

[00:18:31] yeah so uh i think the hardest thing is trying to it's like the different shades of gray

[00:18:38] and uh you know like the textbooks are like you know the pulp chamber floor is different than the

[00:18:45] pulp chamber wall and the orpuses are at the corner of the you know there's like that the article from

[00:18:52] it's not uh chrasn and rank all it was something else but the mouse holes are at the corner of the

[00:18:58] uh of the uh of the uh the tooth yeah the word excuse me but those are really hard things to try

[00:19:06] to figure out when you're just trying to get into a tooth and not make extra holes sir like

[00:19:10] yes especially if you're cutting through like a pfm or something like that where you can't see

[00:19:14] you know i mean that's tough so one of my biggest things is that'll do is oh rinse so i'll make

[00:19:20] sure that the especially as if you know if you don't have high medication or highlight trying to

[00:19:26] make it a the access a little more of like a inverted coat and not inverted comb but sort of like

[00:19:32] a taper preparation in so you try to let as much light in as possible yeah yep that's one and

[00:19:38] then the other one is just switching to um a slow seed burrow which is a months burrow or uh

[00:19:46] I don't know if you guys you've been using those yes yep i'm familiar with the months burrs they're

[00:19:50] very good yeah nice to have yeah that is do you use them routinely it's not routinely um only on

[00:19:57] the ones there's some that you're gonna be able to find the canal yeah like i mean you know yeah so

[00:20:03] it's only if i can you know those ones you look at in your extra like man this is i'm in for it

[00:20:08] here like those that's when you break out your months burrs but there's some you know like you're

[00:20:11] doing a you're doing a root canal like someone who's you know under under 35 i mean it's gonna be uh

[00:20:16] yeah you know you're not gonna you're not gonna need those months burrs your four rounds

[00:20:20] gonna get the job done so that's what i would do what Kevin do you how often to use your months burrs

[00:20:24] well it's um it sounds like you're saying um monster so i'm thinking about herman month

[00:20:30] months burr would be a great name for a dentist uh who is herman months frankenstein but uh yeah

[00:20:36] it's some of those it's weird you get into the some of those that are gonna look difficult and then

[00:20:41] you just fall right in and you think what was i so worried about and there's these other ones where

[00:20:46] you worried about it ahead of time and then you actually do need to worry so i just have them handy

[00:20:50] in case i need you know that it's that situation that's amazing i love to hear that you guys have

[00:20:56] them honestly yeah such a damn secret for a long time now i've got actually to wind up that game

[00:21:01] there's actually from um this german gentleman uh commented that they from comic burrs they're

[00:21:08] called endotrasers hmm and they're half the price oh exactly that's my language that's

[00:21:15] that's an hour and a half we're talking i love them so i have them on order um yep

[00:21:20] so you get them through uh comment in the states there 35 bucks for four or five versus like 63

[00:21:27] for a month and uh they're the same it little appear and then the brochure look appear to be

[00:21:33] the same dimension and same length same everything just cheaper love it i've got a comment rep that's

[00:21:38] a good friend of mine so she'll be happy to know that um that was mentioned here on the podcast

[00:21:43] yeah the endotraser if you're out there where uh we're uh you know we're on your side there

[00:21:48] there we go it just i look i can't i'm actually waiting for the show up uh to use these damn things

[00:21:56] because i've because i'm an international uh dentist to get those burrs across the border is like

[00:22:05] uh fairly expensive it's like smuggling whiskey or something yeah exactly do you i didn't even think

[00:22:12] about that aspect of it like uh but so what files do you like uh are it is like edge endo in your

[00:22:20] area or do you have to use like dense supply or who whose files whose uh roadries do you use for your

[00:22:25] your average cases these days so you know what we train uh what i teach and what i've been using for

[00:22:31] the last since they got out was uh wave on gold that's okay yep pretty much what i've been using

[00:22:37] i've found i've only broken in about four thousand cases i've broken about three or four of them

[00:22:41] and they were just total shouldn't have been doing that with it but yeah uh honestly like for everybody

[00:22:49] my heart is like if you want to make it life easy just use those things and just be done like

[00:22:55] don't reuse them yeah yep that's what i found like and the reason why we do that is because we've

[00:23:01] done so many extractor teeth with the same file like over it over and over and over and over and

[00:23:06] over like man these things don't break so that's kind of why we're kind of really i use them in

[00:23:12] our brand new dentist use them just to stand it's to standardize everything and then also just kind

[00:23:18] of try to have that it's cheaper not to break something so that's kind of sure i don't know what

[00:23:23] you guys using i've always been on pro taper and then pro taper has been you know off patent for

[00:23:29] so long the edgon there's it's on there's a million different edge files there's even

[00:23:34] there's off patents of those off patent one spot i mean everyone copies that that system

[00:23:38] and kevin you're on the etx um agendophiles Buchanan's file yeah wow cheat cheese yeah

[00:23:47] etx yeah that's right so you brought up a good little segue ash and that is like you talked about

[00:23:56] doing extracted teeth yeah and teaching your new dentist so i have long been a proponent that anyone

[00:24:04] who doesn't do endo like you can spend a whole bunch of money on a course or you or you can line

[00:24:13] up about 50 extracted teeth and use and i've linked a bunch of people to your tabletop endo video where

[00:24:20] you put the teeth in the in the alginates that you use your apex locator and i swear if you sit down

[00:24:34] what yeah tell me if is that how you teach people these days so with the system uh in the organization

[00:24:40] that i work um we have a clinical course so it'll be we take six students there's three instructors

[00:24:48] and uh we'll take them tabletop for the first week this is what i mean i was taught this this is

[00:24:52] how i was taught yeah tabletop for the first week clinical for the next so what what i what we have

[00:24:59] learned as instructors is that i'd say 50 to 60 percent of the problems are eliminated by doing

[00:25:07] this strict tabletop so they're doing extracted teeth for the week the first week and then

[00:25:12] then it's just patient management issues for the second week so we walk them through and usually

[00:25:17] it's like hot teeth there's stuff like that right so it's not getting bogged down in how to use

[00:25:23] your apex locator while somebody's you know there's somebody in the chair it's like they've already

[00:25:27] learned how to do that and that what blows my mind is that how even our students that just got

[00:25:34] gradually gradually last year are not learning how to use their apex locator properly before the

[00:25:38] patients in the chair right yeah so simple yeah um you have some good videos on on apex locators now

[00:25:46] is one of the best things i i got better at my apex locator after using your uh put the tooth

[00:25:52] and allogeneity you know technique and and just like practicing with the apex locator and like i

[00:25:57] said i'm with you that like if you sit down on a tabletop and use the same file system and go

[00:26:02] through those the reps a bunch of times and you maybe you break it a couple times you get to feel

[00:26:07] like when a files thinking about breaking you get you kind of get that feel like that's that's

[00:26:13] endo right there know your system and know its limits absolutely that that's that's that's that's

[00:26:19] big i think i think just to add on to that i think what it as well it's it's confidence building

[00:26:26] but it's also like i just thought about that as you said it like when it breaks you know 15 years

[00:26:33] ago if something broke i'd be like oh my gosh what are we gonna do but now it's kind of like you

[00:26:39] know you're you broken a bunch in extractives he's broken some and people you know what to say

[00:26:45] to the person you know how to manage it and you know you know the literature is pretty solid on

[00:26:52] if it's a vital tooth and it's broken it's probably gonna be okay yeah excuse me it's the

[00:26:57] chronic well then it just depends on you know how much bleach has been down there for how long and

[00:27:01] what not uh but just having that confidence like yeah you know i broke it last time i know what

[00:27:07] to and sit the patient up or finish the case and be like okay well we're gonna follow you up or

[00:27:10] whatever you need to say to them right just that confidence building i think one of the benefits

[00:27:15] of practicing for so long is you know you're gonna break a file once or twice a year times 30 years

[00:27:23] and a lot of those teeth have never come back or had a pro or had a big problem so that's true it's

[00:27:30] it's not the tragedy that you think it is or that you're taught it also though all of us have been

[00:27:38] in practice long enough to know that god bless the people who came up with heat treated files

[00:27:43] yeah because like it's a lot less of a problem now than it was you know 10 years ago when we were

[00:27:51] just on the non-heat treated files for sure i mean Ash what do you notice and you do a lot more

[00:27:56] canals than both of us combined you notice them between you and your students less breaks overall

[00:28:02] since since heat treating was brought into the system yeah i think definitely heat treated has made

[00:28:09] a significant difference and one of the biggest things i can that comes right on my mind is getting

[00:28:15] you know not so much about when it's in the canal it's actually getting it into the canal

[00:28:21] you can actually bend the hell up to get it in the field yep yep love it yep oh my gosh you know to get

[00:28:30] the students to just they're like you know they kind of like they do the little the little like

[00:28:35] banana curve and like listen just give it a good huge bend because it's make your life easier to

[00:28:40] get it snagged into that musiobuckle yeah i've been able to muller that's a huge tip that i've learned

[00:28:46] it's so many years of finally bend the hell out of it it's not gonna hurt it no

[00:28:52] just to get it in there yeah much much better much much easier access with something like that

[00:28:57] what are what are some other what are the other things that like what are some of the things that

[00:29:01] like students miss the boat on that like you know it's experienced guys like we know those tips and

[00:29:06] like we take them for granted what is what's another like what's a big one the students miss out on

[00:29:11] yeah so that's a great question actually um i just we have an internal Facebook for where i work

[00:29:18] and i was just uh coaching just literally last week and uh my dentist or dental or dentist was

[00:29:28] she's but three years out she was super excited because she's using the scope i'm recording it

[00:29:32] and she takes she's like watch so she the dental system can watch through the my my computer

[00:29:38] what's going through the camera so she's like watch yep and she aspirates out of the musiobuckle one

[00:29:44] and it you know it drains from the mb2 right so it's like look they connect and i'm like well

[00:29:53] 15 years ago i used to think that and yes they do connect but i've been burned by not

[00:29:58] cleaning and shaping one of the canals fully because there's like 85% of time there's an itzmus

[00:30:06] yep that connects those two yep and i'll tell you man i was burned and i was just like why didn't

[00:30:12] you know it's like oh well yeah it's miss so that was you know that's one so it's it's

[00:30:18] so it's not just relying on that technique if you want to make sure that they join so we will go

[00:30:22] down the next iteration of okay cleaning shape one of those canals take another like 10 or

[00:30:27] you know 15 or 20 file hand file put a cut of perch in one piercet with the other canal

[00:30:34] and then pull your got a perch out and see if there's actually a score mark or piercing

[00:30:39] so just to check because then so say for example you're having a heart it's a pretty

[00:30:45] pretty practical way of doing it you get down you know you get down your musiolingle kit out of your

[00:30:48] max or your mendever they're bowler which is usually a little bit straighter and you just can't

[00:30:53] get around that mb you're just like the last two millimeters like dammit you know you just keep

[00:30:58] yes you keep pushing files and you just keep bending you like you know it took me so long to not do

[00:31:04] that because you just want to force it yeah just like yep and so one quick way to can do that

[00:31:12] is just you know shape your the musiolingle to whatever file you're doing and then place you've

[00:31:17] got a perch in and then take whatever size file you can get down your musiobackel and see if

[00:31:22] it actually hits it somewhere yes because if it hits it and you can pull a got a perch on you like

[00:31:29] well i'm trying to you know i'm trying this file is coming in at 90 degrees into this musiolingle

[00:31:35] maybe i shouldn't try to make it go another 90 degrees and bend out so we can

[00:31:40] you'll break it yeah yeah yeah you'll just break down file if you try to do that kind of stuff

[00:31:43] and then you're still frustrated like why did it break like well you're trying to force it around

[00:31:47] a corner didn't want to go but you could have been way ahead of the game you know so

[00:31:52] that's one where i had to learn many times the hard way

[00:31:56] oh yeah i've got a few of those hard knocks too i'm sure

[00:32:00] life right life's a teacher like that that's for sure that is for sure oh man you know all right

[00:32:07] oh i know what i'm going to talk about yeah you know one of the things that's really been on my mind

[00:32:10] so i've been listening to the the American Association of Internonist I attend their conference

[00:32:16] in this year's virtual and it'll be interesting putting in the comments because there was a big push

[00:33:44] even an issue because of the the multi sonic nature of being able to eat clean
